re:HEALTH Terms & Conditions:

1. The promotion period is from now to August 18, 2025 (“Promotion Period”). Treatment offers at re:HEALTH (the “Outlet”) is only applicable to selected items. (the “Treatment Offer”).
2. “Standard Health Check Plan”, “New Generation SHINGRIX Shingles Vaccine 2 Shots ” and “9-valent Cervical Cancer Vaccine 3 doses” offers are applicable to the new Customers of the Merchant.
3. “Influenza vaccine” offer is applicable to the existing Customers of the Merchant.
4. Customers must provide promo code T1367 to make a booking with the Outlet via WhatsApp (852) 6450 0900 or designated online registration form.
5. Customers shall present and pay with their Citi credit card upon arrival at the Outlet to enjoy the Treatment Offer.
6. Customers registering for the service must provide their identity card number for booking and verification purposes.
7. Each Customer can only enjoy the “Standard Health Check Plan”, “New Generation SHINGRIX Shingles Vaccine 2 Shots” and “9-valent Cervical Cancer Vaccine 3 doses” offers once during the Promotion Period.
8. The Customer should understand and agree to the items and content of the Treatment Offer and the services arranged by the Merchant and/or the Outlet before receiving the same.
9. The Merchant shall be entitled to revoke, alter, add to or vary the contents used in the Treatment Offer without further notice.
10. In case of any dispute in respect of the Treatment Offer, the Citibank and the Merchant has the final decision.
11. Citibank shall not be responsible for any matters in relation to the related products or services. The respective merchants are solely responsible for all obligations and liabilities relating to such products or services and all auxiliary services.
